WHEN I GROW UP - COACHING AGREEMENT
Client and Coach acknowledge and agree that:
Coaching is

A partnership (defined as an alliance, not a legal business partnership) between the Coach and the Client.

Coaching services are designed to facilitate the creation/development of personal, professional, or career goals and to develop and carry out a strategy/plan for achieving those goals.

Coaching includes no guarantees or warranties, expressed or implied, about any results to be achieved.
Coaching is not

Coaching is not therapy, counseling or advice giving and is not a substitute for psychotherapy, psychoanalysis, mental health care, substance abuse treatment, legal,
medical, or other professional services. -
Coaching does not prevent, cure, or treat any mental disorder or medical disease and it is the Client’s responsibility to seek such independent professional guidance as needed.

Client understands that in order to enhance the coaching relationship, the Client agrees to communicate honestly, be open to feedback and assistance and to create the time and energy to participate fully in the program.

Client acknowledges that coaching is a comprehensive process that may involve different areas of his or her life, including work, finances, health, relationships, education, and recreation. The Client agrees that deciding how to handle these issues, incorporate coaching principles into those areas and implementing choices is exclusively the Client’s responsibility.
Responsibility

Coach will maintain the ethics and standards of professional behavior established by the International Coach Federation (ICF). The ICF Code of Ethics is available here: https://coachfederation.org/code-of-ethics.

Client is responsible for personal, physical, mental, and emotional well-being and is solely responsible for decisions, choices, actions, and results.

Client agrees to inform Coach of past or current mental health or substance abuse conditions, including concurrent treatment with other professionals.

Coach is not and will not be liable or responsible for any actions or inaction, or for any direct or indirect result of any coaching services provided.
Confidentiality and exceptions to confidentiality

Coach will keep all information and records confidential, to the extent defined by the laws of the state/country in which Coach and Client reside.

Coach will not disclose any information pertaining to the Client without the Client’s written consent.

Coach will notify appropriate mental health professionals and/or authorities if Client represents a danger to self or others.

Coach may be a mandated reporter of child and/or elder abuse.

Coach may share topics with other coaching professionals for training, supervision, and mentoring for professional development purposes divulging any identifying Client information.

Coach and Client acknowledge that communications via text and email are not encrypted nor secure, and therefore may not be confidential.
Services
The parties agree to engage in a single session Coaching Program through internet video meeting.
Schedule & Fees
The fee is $150 USD.
The calls/meetings shall be 45 minutes.
Procedure
The time of the coaching meetings and/or location will be determined by Coach and Client based on a mutually agreed upon time. The Client will initiate the scheduled call and will log on to the provided video link by the Coach for the scheduled meeting.
Release of Information and Session Recording
The Coach engages in training and continuing education pursuing and/or maintaining ICF (International Coach Federation) Credentials. That process requires the names and contact information of all Clients for possible verification by the ICF. By signing this agreement, you agree to have only your name, contact information and start and end dates of coaching shared with ICF staff members and/or other parties involved in this process for the sole and necessary purpose of verifying the coaching relationship, no personal notes will be shared.
Additionally, as part of ongoing training and mentoring, Coach may record client session for the purpose of mentor feedback and/or professional growth.
According to the ethics of our profession, topics may be anonymously and hypothetically shared with other coaching professionals for training, supervision, mentoring, evaluation, and for coach professional development and/or consultation purposes.
7) Cancellation Policy:
Client agrees that it is the Client's responsibility to notify the Coach 24 hours in advance of the scheduled calls/meeting. Coach reserves the right to bill Client for the missed meeting. Coach will attempt in good faith to reschedule the missed meeting.
Your agreement indicates full informed consent, acknowledgement of, and agreement with the information outlined above.
